Diabetes Annual Review
All patients with diabetes should have their annual review that involves blood tests, measurement of height and weight, urine check, blood pressure and foot check. It will also include your medication review and discussion of your general health and psychological wellbeing. This will be done during a routine appointment.
In addition, the annual review you should have had examination of your eyes -Retinal screening at Honeypot Lane.
Brampton Health Centre provides additional services to help and support patients to have access to specialist care to achieve individualise management care plan.
Diabetes Specialist Clinic
The clinic is ran by the Diabetes Specialist; Nina Patel. The clinics are for patients with complex and those requiring additional support in order to achieve their personal individualise target. If you need to see the Diabetes Specialist Nurse – call and request for an appointment. The Clinics ran once monthly.
Diabetes Education Group
This is an in-house service ran by Dietician and Diabetes Specialist Nurse.
This is a group session, delivering education about diabetes care and management for 2 hours.
It is available to anyone who likes to have a further understanding about Diabetes or meet other people with diabetes.
If you had diabetes for 10 years or 5 days, a Carer or family member and would like to attend please call the reception.
This is a new popular service to Brampton ran every month -call reception and request for Diabetes Group Education Service. It is ran once monthly in a small group of 7.
Diabetes Structured Education
DESMOND is a one day course, offered on days to suit you. Each course is attended by about 10-15 people with type 2 diabetes. The local venues for DESMOND in Brent are; Monks Park Primary Care Centre, Willesden Centre for Health and Care and Wembley Centre for Health and Care – referral can be done by any Clinician at routine consultation.
